We help manage the complicated maze of social service choices.

Service coordinators assist individuals with disabilities and their caregivers in accessing services, which focuses and impacts the individual’s life. This includes living arrangements, in-home services, education, employment or employment programs, day program, recreation, personal needs, transportation, medical needs etc. The family may request services such as respite, guardianship, caregiver support group, etc. The ultimate outcome of this planning is to assist the individuals in becoming as independent as they are capable of becoming and to assist the family/caretakers in achieving these goals with their disabled child/or dependant.

Medicaid Service Coordination is a Medicaid based program in which service coordinators provide assistance and advocacy to program participants. Service Coordinators implement an Individualized Service Plan and help access a wide variety of services including medical, vocational, residential, respite, and recreational. Service Coordinators ensure that services are provided in accordance with the Individualized Service Plan. This program is available to residents of Queens, aged 4 or older who have a documented developmental disability.

Non-Medicaid Service Coordination is a support service for individuals who do not have Medicaid benefits. Service Coordinators assist in applying for financial entitlements and the Medicaid Waiver, as well as assist in accessing a variety of services, such as socialization, clinical, and respite. This program is available to residents of Queens, age 4 or older who have a documented developmental disability.
